New service “Kilburn Care at Home”

The practice is now offering an exclusive service for patients aged 65 and over, called “Kilburn Care at Home”.  From now on, 7 days a week 8am to 8pm patients requiring an enhanced level of care can access GP advice more quickly and easily than before.

The aim of the service is simple, to make sure that you receive the right care in the right setting.

How do I access the service?

If you are 65 and over with one or more long term condition and have difficulty accessing care outside your home, you may already be registered for the service, in which case accessing it is easy: simply dial 111 when you need GP advice and we’ll be able to respond to your need as appropriate.

To find out if you are eligible or to register for the service please contact us on 020 3667 5564

Please note that this service is not to be used in an emergency. If you require emergency medical attention please dial 999.

As this is a new service your feedback is very important to us and we would like to know your experience of using it, please email us at Kilburn.admin@nhs.net or telephone 0333 321 2084.

If you wish to opt out of the service please contact the practice on to discuss 020 3667 5564.
