New Patient Registration
We welcome new patients who live or work within our practice area. We register new patients from our catchment area of Brent, Westminster and Camden.
All new registrations would be required to have a medical with our Health Care Assistant (HCA)to discuss past and present conditions and a urine test will be performed. For children under the age of sixteen, parents will be required to provide their child's Red Book (record of their immunisation history).
You can use the link on this website to complete a registration form or collect a registration form (GMS1) from reception. You will also be required to complete a health questionnaire.
Please contact reception for further information and to book an appointment with our HCA. Once you have had a medical with the HCA you will then be registered at this practice.
The Royal College of General Practitioners has produced a useful guide for patients about the services on offer at GP Surgeries and how to access them. You can download the guide below.
