New Patient Registration
We welcome new patients who either live or work within our practice area. We register new patients from our catchment area of Brent, Westminster and Camden.
Together with the registration form, you will be required to complete a New Patient Health Questionnaire together providing proof of ID and address.
All new patients over the age of 40 are required to have a medical with our Practice Nurse/ HCA to discuss past and present conditions and a urine test will be performed.
For children under the age of sixteen, parents will be required to provide their child's Red Book.
Please contact reception for further information and to book an appointment with our practice nurse. New registration appointments can be booked up to two weeks in advance.

A Patient Guide to GP Services
Required ID:
We would appreciate proof of address with the patient’s name on it dated within the last three months. Please note: patients new to the UK without a valid visa may be charged for services in the NHS i.e. Hospital treatment
- Passport with valid visa
- Bank statement
- Utility bill i.e. Gas, electric, water rates
- Credit card statement
- Correspondence from unemployment benefit office
- Electoral register
- Telephone bill
- Dss order book / letter
- Council tax bill
- Council rent book / private rent agreement
- Mortgage statement
- Home office letters
Students 16 to 19
- Proof of parents’ address
- Plus one of the above
- Under 16’s are exempt from proof of address
