Administrative Requests

If you require a letter or form completed, please complete the consent form at reception. Please note, all documents/letters completed by the GP is at their discretion and will take up to six weeks to complete. For further information speak to reception. 

Please allow 48 hours, excluding weekends and Bank Holidays, for your request to be processed. However, if you request a prescription using your unique log in through Patient access, your prescription will be sent to your nominated chemist within minutes / hours. Any problems please telephone the surgery after 2pm.
